Summary

The PACT Social Work Supervisor is responsible for the professional and administrative management of the social work service for Primary Care in the Orlando VA Medical Center and supervises the social workers assigned to the PACT.

Duties

Duties Include: As the PACT Social Work Supervisor, the incumbent provides both administrative and technical supervision to the social work staff as assigned. The incumbent is expected to set priorities and make assignments according to capabilities of employees and oversee personnel management functions of staff assigned to PACT teams. The incumbent evaluates PACT organizational metrics and collaborates with leadership on continuous performance improvement regarding care coordination. The PACT Social Work Supervisor is responsible for monitoring compliance with social work practice standards and guidelines on documentation, workload, data entry, ethical practice, and service delivery. The PACT Social Work Supervisor will complete all required supervisory duties to include Time and Leave (T&L) for assigned staff and assigned back-up T&Ls. He/she will coordinate with Social Work leadership and Human Resources on hiring and/or disciplinary actions and complete annual evaluations for assigned staff. The incumbent will independently complete psychosocial screenings and assessments; develop treatment plans with goals and objectives, provide support and counseling to family members; make referrals to other service providers, VA and community programs; and works collaboratively with the multidisciplinary team to provide care to veterans. The incumbent performs other related duties as assigned, including coverage in other social work areas. The incumbent documents independently in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up, and referral. Documentation includes but is not limited to, initial evaluations, suicide risk assessments, group notes, care coordination, case management, advanced directives, clinical reminders or initial treatment plans. All clinical interventions, whether by telephone or face to face, will be recorded in electronic progress notes. This information will be entered in a complete, confidential, and professional manner. Required documentation will be completed and signed in the timeframes set by facility policy or national program requirements. Workload will be recorded in accordance with VHA policy and local medical center guidelines. Other, various duties include the participation of contributing to the successful completion of clinical reminders as appropriate for Social Work scope of practice and/or privileges and performance measures of the facility as assigned. Work Schedule: Monday-Friday, 8am to 4:30pm Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
